Cancellation Policy

To cancel or reschedule an appointment, please contact the office at least 24 hours before your scheduled appointment to avoid cancellation or no-show fees. Patients who do not adhere to the cancellation policy will be charged half of the cost of the missed office visit.

Payment

Payment is expected at the time of service. Master Card, Visa, check or cash are accepted. If you have insurance that covers acupuncture, a receipt for your claim will be provided.

New Patients

New patients please fill out the new patient forms (PDF, 164k) and bring them with you on your first visit.
